Care Coordination & Health Navigation (with Interpreting)
Get the right help, first time— with interpreters and digital support on hand. Our Care Coordinator and Health Navigators work with you in the waiting room, by phone or online to book the right appointment, sort referrals and complete forms. Interpreters are available, and we use simple tools to keep things moving.

Interpreting—your way
We work alongside Sirona Health Link interpreters. For quick, simple conversations we can use Pocket Talk—a small, handheld two-way translation device that translates spoken phrases on screen and aloud in real time. We use Pocket Talk for short, practical exchanges (check-in, directions, form prompts). For detailed clinical discussions or consent, we arrange a trained interpreter so nothing is missed. If you prefer, ask us to book a longer, language-supported slot.
Digital help while you wait
Bring your phone—we’ll help you use our website forms and the NHS App, step by step. If you’re new to smartphones or online services, we’ll go at your pace. We also have a Patient Online Access Zone in the health centre with a PC you can use to access online healthcare services.
Privacy, dignity and choice
If you’d prefer to speak with a female or male staff member, tell us and we’ll do our best. Our team responds with compassion, and we prioritise continuity with a clinician you know. If you feel overwhelmed, we can move to a quieter, private space.
