Learning Disability & Dementia Annual Health Check Clinics
The same faces, extra time, and adjustments that work for you. Our annual health check clinics are led by named care coordinators and nurse practitioners, with follow-up—wherever possible—with your usual GP or their micro-team. The aim is to reduce anxiety, build trust and make each visit count.
Who the clinics are for
Adults on our Learning Disability register and people living with Dementia—plus families and carers—who benefit from steady relationships, longer appointments and clear, sensitive communication.
What to expect
Before your appointment. We ask what helps: preferred language, who you’d like with you and any access needs. We can arrange an interpreter (and use Pocket Talk for quick exchanges), book a quieter slot, and make sure you see familiar staff. On the day. A known nurse practitioner and care coordinator lead your health check—observations, long-term condition review and medicines—plus anything due such as screening or immunisations. If you’ve come in for something else, we still make every contact count and offer what’s due while you’re here. Afterwards. We agree a simple plan, set reminders in the way you prefer and—where possible—book follow-up with your named GP or micro-team, so you’re not starting from scratch next time.

- Communication support: interpreters on site; Pocket Talk for brief, practical conversations; information in a range of formats.
- Continuity by default: we keep you with clinicians who know your story.
An environment designed for access
Our waiting room was co-designed with patients and accessibility partners including Bristol Dementia Alliance and RNIB. It includes a hearing loop, clear signage and a quieter area—small changes that make visits easier.
Community partners who help us reach you
We work with trusted VCFSE organisations—including Caafi Health and Nilaari—and with local pharmacists to widen recalls and health checks in familiar community settings.

Tell reception you’re booking a Learning Disability or Dementia annual health check. Let us know any adjustments you’d like—language, room, longer time—and who will attend with you. We’ll do the rest and aim to keep you with your usual team.
